How to complete a direct rollover
If you are looking to complete a rollover to move out of your old workplace retirement account, we can help you complete this. See the steps below to get started:
How to complete your direct rollover
- Call your old workplace retirement accounts Custodian(s) (you should find a customer service number on a recent statement available in paper or online)
- Mention that you no longer work there and that you have an IRA or Solo 401(k) with Altruist set up to accept direct rollovers (your advisor will let you know which to transfer to)
- The custodian representative will either:

Take your information and generate a Direct Rollover check made payable to Altruist with your Altruist account number in the memo. (To find your account number, see this guide: slpwealth.helpscoutdocs.com/article/161-how-do-i-find-my-account-number )
OR
- Ask for your mailing address to send you forms or the rollover check.
If you receive the rollover check, double check that it is made payable to Altruist LLC for the benefit of Your Name and in the memo your account number at Altruist should be listed. If not listed, write it in on the memo line.
*If the rollover check is made payable directly to you - instead of to Altruist for your benefit - then it must be received by Altruist within 60 days of being issued to avoid tax penalties.
For both options of step 3, the check should be sent overnight mail to the following address:
